Salary Deduction Change or Cancellation Form

Purpose of the Form This form is used by employees of the Government of Barbados to request changes to an existing salary deduction authorization or to cancel such authorization. It ensures that deductions from your salary are updated or discontinued according to your instructions.

Employee Declaration By signing and submitting this form, you confirm that:

• The information you provide is accurate and complete.
• You authorize the Government of Barbados to amend or cancel your current salary deduction authorization as specified.
• If you provide revised deduction details, you authorize the Government to adjust your salary accordingly and remit the deducted amounts to the recipient you have named.

• Any disputes regarding debts or obligations must be resolved directly between you and the creditor/payee.

1. Fill in all required personal and deduction details clearly.
2. Indicate whether you are requesting a change or cancellation of your deduction.
3. Provide updated recipient information if applicable.
4. Sign and date the declaration to confirm your authorization.
5. Submit the completed form to your Ministry’s HR or Payroll Department.
